A licensed and credentialed Palliative Care Program Nurse
Practitioner (NP) provides diagnosis, treatment, consultation and
follow-up under the supervision of 1 or more physicians. The NP
provides age and specialty appropriate medical care at the level of
training achieved. A Nurse Practitioner is an extension of a
physician, being able to provide full hospice across all patient
types. Conduct complete physicals, provide treatment and counsel
patients. Order and interpret test as indicated and recommend
treatment of patients. Administer therapeutic procedures. Prescribe
therapy or medication with physician approval. Nurse Practitioner's
work within a defined specialization which can be any type of
medical care, including, but not limited to: adult nursing,
community health nursing, family nursing, geriatric nursing, &

and families to identify the physical, psychosocial and
environmental needs of patients as evidenced by documentation,
clinical records, IDT reports, after hours reports , and on-site
evaluations. Meets all regulatory and accreditation requirements
related to the Plan of Care.

chronic disease management issues of the population in the practice
while administering quality patient care.

achievement of patient outcome.
- Demonstrates competency in all levels of care for Hospice.
- Maintains legible, accurate, and confidential medical records.
Documents all medical evaluations, diagnoses, procedures,
treatment, outcomes, education, referrals and consultations
consistent with state regulatory standards.
- Educates patients and/or families about hospice care, medical
issues, and use of prescribed medical treatments and/or

quality standards according to protocol and receives and implements

coordination of services with physician, facility staff and team

- Serve as staff liaison with nursing facility, medical
directors, director of nursing, attending physician and community

this knowledge into clinical practice.
- Promotes the Agency philosophy, mission statement and
administrative policies to ensure quality of care.
- Maintains patient & staff privacy and confidentiality at all
times pursuant to HIPAA Privacy Final Rule.
- Provides positive, supportive communication to physicians,
patients, families, facilities, visitors, and other agency

EDUCATION: Graduate of an accredited Certificate, Diploma,
Associate or Baccalaureate School of Nursing.LICENSURE: Current
State License as a Registered Nurse. Certification as a Nurse
Practitioner.EXPERIENCE: 2 years experience within the last 5 years
in chronic disease management/long term care. Hospice background
preferred.SKILLS: Nursing skills as defined by accepted nursing
standards. Demonstrates ability to assess and respond to the needs
of patients, families and caregivers in varied settings and to
function effectively in the home or nursing facility environment.
Demonstrates skill in nursing practice, ability to cope with family
emotional stress and tolerance of individual lifestyles. Ability to
handle multiple priorities, documentation requirements and
deadlines.TRANSPORTATION: Reliable transportationHEALTH STATUS:
Meets all applicable agency policies and procedures related to
health screening and required testing.
Physical Requirements/Working Conditions:1. Complete required work
on a daily basis.2. Cope with work involving ill, disabled, and
dying patients.3. Sit, stand, bend, lift, and move intermittently.
Must be able to lift at least 25lbs and bear the weight of an
average adult effectively so as to be able to perform the above
listed functions.4. Move equipment and supplies necessary for job
functions.5. Read in a 12 point or larger type, speak and write
English in order to fulfill job functions in an understandable
way.6. See/hear or use prosthetics that enable accomplishment of
job.7. Meet the health requirements of the agency.8. Possible
exposure to infectious diseases.9. Abide by all occupational safety
and bloodborne pathogen protocol to minimize illness and/or
injury.10. Move, support and/or transfer patients as required to
perform essential functions of position.
