DEPARTMENT: Nursing ServicesWORK AREA: Entire FacilityPURPOSE:Responsible for coordinating all activities of the Infection Prevention and Control Program that meets the (CMS) Centers for Medicare and Medicaid Services requirements including, but not limited to, areas outlined below. Supports antibiotic stewardship initiatives and is an active member of the facility (QAPI) Quality Assurance Performance Improvement Committee.THIS PERSON REPORTS TO: Director of NursingWORKING HOURS: Must be employed at least part-time by the facility.HIPAA MINIMUM NECESSARY:Responsible to adhere to HIPAA MINIMUM NECESSARY guidelines and safeguard protected health information.WORKING CONDITIONS:Well-lighted, well-ventilated office area and rooms. May stand, stoop, bend and walk intermittently during the work day. May have to deal with emotional disturbances of residents, residents family and staff. May be exposed to infectious diseases and odors, personality peculiarities of the aged. Required to follow the No Lift Policy.DUTIES:Adheres to and enforces White Oak Management Infection Control and Prevention Program.Sets standards and evaluates infection prevention practices and outbreak investigations within the facility.Provides/ oversees staff education during orientation, annually and as needed on an ongoing basis.Provides/oversees resident, family and visitor education as indicated.Identifies and documents infections through surveillance process.Determines need and type of transmission based precautions to be initiated and discontinued.Notifies public health department of reportable diseases.Supports and oversees Antibiotic Stewardship Program and initiatives.Works closely with other departments to reduce the risk of cross contamination in the work environment and strengthen infection prevention practices.Develops action plans to address opportunities for improvement in staff and facility practices.Coordinates employee health concerns with human resources and department leaders.Conducts the annual facility Infection Control Risk Assessment process.Works with community partners (i.e., Mental Health facilities, Hospitals, Assisted Living facilities and other Long Term Care facilities) to help reduce the risk of transmission of infection across the continuum of care.Makes routine infection prevention and control rounds throughout the facility on a regular basis.Works closely with licensed nurses and attending physicians to ensure compliance with established protocols related to initiation of antibiotic therapy.Completes infection prevention and control and antibiotic stewardship reports on a monthly, quarterly and annual basis and presents to QAPI Committee for review and input.Monitors schedules for routine cleaning of entire facility as well as terminal room cleaning, cleaning of equipment, etc.Oversees staff competencies related to infection prevention and control on hire, annually, and on an as needed basis.Other duties as assigned.by Jobble
