Job Description:
Physical Therapist - Long-Term Acute Care (LTAC) If you are
passionate about helping patients recover from severe illnesses or
injuries, join us as a Physical Therapist in a Long-Term Acute Care
(LTAC) setting. In this role, you will provide specialized therapy
services to improve mobility, strength, and overall functionality
for patients requiring extended care. Key Responsibilities: -
Evaluate patients' physical conditions and develop individualized
treatment plans tailored to their recovery needs. - Create therapy
programs focused on improving mobility, strength, coordination, and
pain management during long-term care stays. - Collaborate with
interdisciplinary teams, including physicians and nurses, to ensure
comprehensive patient care plans are implemented effectively. -
Educate patients on exercises and techniques that support continued
recovery after discharge from the LTAC facility. - Document patient
progress accurately and adjust treatment plans as needed based on
clinical outcomes. Work Environment: - The role is based in LTAC
hospitals where physical therapists help patients recovering from
surgeries, injuries, or chronic conditions requiring intensive
