Behavioral Health needs; care management skills including
assessment, care coordination, and discharge planning Description:
We are seeking an experienced RN - Case Manager to join our team.
The average census and case load per day is approximately 14-16
patients, depending on the unit assigned. The patient population
includes adult medical/surgical, cardiac, and orthopedic cases.
Support within the department includes a director, managers, and a
peer case management team. Technology used includes an EMR system.
The role may involve floating to other units as needed. Weekend
rotation will depend on staffing requirements, and travelers will
work all holidays unless approved off in the contract. The
department culture is very supportive and teamwork-oriented.
Educators are available to support staff through changes. The ideal
candidate will possess strong care management skills and experience
working with diverse patient needs.
