Job Description:
Agape Care Group, a leader in providing compassionate hospice
and palliative care across the Southeast, is seeking a dedicated
Nurse Practitioner to join our Palliative Care team. In this role,
you will have the unique opportunity to make a profound difference
in the lives of patients and their families during their most
profound moments. You will work closely with a team of medical
professionals to deliver personalized, patient-centric care aimed
at improving quality of life and providing comfort and dignity to
individuals facing serious illness. Conduct comprehensive
assessments of patients to determine their physical, emotional, and
social needs. Develop and implement personalized care plans in
collaboration with other healthcare team members. Provide direct
patient care, including managing symptoms, prescribing medications,
and performing procedures within the scope of practice. Educate
patients and their families on disease processes, treatment
options, and palliative care principles. Collaborate with other
healthcare professionals to ensure a coordinated approach to
patient care. Regularly review and adjust care plans to meet the
evolving needs of patients. Participate in interdisciplinary team
meetings to discuss and optimize patient care. Strong clinical
skills in palliative care and symptom management Excellent
communication and interpersonal skills Ability to work
collaboratively within an interdisciplinary team Compassionate and
empathetic demeanor with patients and families Expertise in
clinical decision making and patient assessments Medical Insurance
