Apply now and a recruiter will connect with you to share further details. /n /n Minimum Requirements /n - Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. - Current Physical Therapist license in North Carolina required to start the assignment. - Current CPR certification required. - Must have a valid driver’s license, current vehicle insurance, and access to a dependable vehicle for patient visits across the service area. - Experience providing home health physical therapy, including patient evaluations, development of plans of care, progress documentation, and discharge summaries. - Ability to document CMS-required functional status and coordinate with physicians and interdisciplinary team members. - Schedule flexibility to participate in a weekend rotation and potential availability on observed holidays as needed. - Ability to complete timely progress notes and evaluations per agency policy and report outcomes to nurse supervisor within applicable timeframes. - Preferred: Experience supervising PTAs and home health aides in accordance with state regulations. /n /n Assignment Details /n - Primary Specialty: Physical Therapist. - Facility Type: Home Health. - Job Type: Travel assignment. - Status: Full Time. - Visits conducted across the agency service area; candidate may be required to travel to nearby agency service areas as needed. - Responsibilities include patient evaluations, development and implementation of PT plans of care, documentation of CMS-required items, progress notes for each visit, participation in weekend rotation, and potential holiday availability. /n /n Location Highlights /n The Sylva, NC area offers mountain recreation and a small-town lifestyle within easy reach of major outdoor attractions.
